Because today movies are made in a way that you can go and piss for 30 minutes in the middle and it will have no effect on your understanding of the movie

THEORY:
Films 2 hours or less definitely don't need an intermission. Films 3 hours or more definitely do need one. The middle area is tricky.
Therefore, films should be <2 hours or >3 hours.

Sadly this theory breaks down with Goodfellas which feels just the right length at 2 hours 26 minutes.

All the same, we should Make Intermissions Great Again.

You could have an intermission and show something soothing and low-key in the intermission for people with capacious bladders who just want to sit there. Waves breaking on the shore, or Sydney Sweeney eating ice-cream or something.

back in the days, everyone had 2-5 STDs so they had to piss more often

It came from operas. If you have to sit through 3 to 4 hours of singing and acting, a bathroom break is necessary. For cinemas this was also good for business, people could go to the lobby and buy some more snacks.

It was used as a reminder to excuse yourself to go delouse and drain your boils.
