The doctor is a simulation and expertise and skills are built upon countless simulations. Like the doctor has experiences of lifetimes of ther doctors from it's modeling. In a very similar way that modern androids using LLM AI are today. Basically Data is an external hard drive experiencing the universe in a parallel way that humans do. The Doctor is basically an AI program user intergace model designed to interact with humans. They are very different. Honestly the scifi show that probably got it most right was probably Red Dwarf with AI and Androids

It's why the holodeck was actually able to make a character capable of going toe-to-toe with Data, which resulted in Moriarty. Which I suspect ended up in the hands of Zimmerman when Moriarty was put into that holo-cube computer by Barclay, who studied it and used the knowledge to make the EMH tech.

With some reprogramming I believe most starships are capable of sentience.And I do think that's the major difference between Data and Holograms. Data is a self contatined unit. All holograms as far as I am aware are dependent on a ships processing power or a device from centuries in the future to hold all the information that allows them to reach a higher cognitive framework or something resembling life. Every gets caught up in the metaphysical quandary of what life is and ignores the physical necessities that make it possible.
