We already got an example of this with the Sons of Sam docuseries and Chaos (on Tom O'Neill's Manson book) docuseries (both of which have similar premises to Programmed to Kill).

Sons of Sam somewhat makes the case for the multiple killers theory, but by the end inexplicably tacks on some statements that suggest the Maury Terry's theories on this were all wrong.

With Chaos, it appears to have just been a smear job. Erroll Morris it turns out is an establishment hack after all and seems to have made the doc just to try to discredit Tom O'Neill's angle on Manson

I think Maury Terry's mistake was just thinking it came down to "satanists" rather than considering that that might been just an element of or cover for some deeper kind of activity (to be fair he had admitted the thought crossed his mind but he couldn't know). The security guard could have been part of some network of shady assets but of course how could we know. Telling the story of the book won't be that hard I think.
Sure Jodge Holden might feel a tad ridiculous instead of ominous. But a good director can achieve that still. Even the ambiguity of The Kid can be somewhat adapted; have him talk with Tobar a bit more/earlier.
More difficult would be the point of violence in the book. There's always a lot of violence, but most moviegoers that want to see violence, want it like Kill Bill, gratuitous and spectacular.
But in Blood Meridian the violence escalates while the impact to those committing it lessens and lessens.
How would you adapt that, without losing the real essence of the book?

Murders of course continue to happen, but the serial killer thing where some guy feels inclined to keep doing it over and over just for the sake of killing, that seems to have cooled down historically

I don't know how many of these cases that David McGowan shows suspicion for in the book had more going on than in the official narratives, but some like the Henry Lee Lucas case and John Wayne Gacy do have some pretty peculiar details and connections

You have to show it entirely from the Kid's perspective, which even within the context of the story is fantastical. Takr the dead baby tree, for example. In movie form, it could be just skulls, but the Kid hears their screams, only to look around and see no one else even flinch. Hell, have the Judge laugh and say something about injuns. The Kid could start to leave, gear the screams, and hesitate for a second until the Judge ushers him forward and speeds up to join the group. Extreme violence, separate from our main characters, and everything you need to know about the group dynamic in underworld a minute.
The style should be across between the dry bleakness of Bone Tomahawk and the fantastical telling of the first half of Natural Born Killers, as seen through the eyes of a kid thrust into an incredibly violent world.

Book of the New Sun by Gene Wolfe. I think a superb adaptation *could* have been made in the early 00s, when CGI made mindbending visuals easily achievable and political correctness didn't strangle directors ability to create relatively true to form film versions of novels.
