Its a play on your assumptions, see you expect a baby rhino coming out, but you already know its jim carrey in a fake rhino, but the family watching it doesnt know that its a fake rhino, so they get surprised by the naked man coming out of the fake rhino. And before this you saw jim carrey getting into the situations that resulted into him losing his clothes and getting moisture on himself, the same circumstances as an actual birth, a naked baby covered in fluids. So it plays on the audiences knowing whats happening that the family is watching thinking the fake rhino gives birth to a real live human. It plays with your expectations because obviously we as the audience know that its impossible for a fake rhino to give birth, let alone an actual human being.
