when people say "they improvised this part!" does that mean that the recorded scene is footage of an improvisation, or that the actors kinda improvised a few lines on the spot because they had better timing, then talked it with the director/writer/whoever and then rehearsed and filmed those changes to the script?

It could mean either. Improvisation just means that something differs from the script. Or something was added which wasn't in the script.

Most of the time it's either: the actor forgot their line so they made up a new one on the spot and everyone agreed they liked it more or the director told the actor beforehand that they could improvise in a certain scene if they wanted to

It can also sometimes describe 'happy accidents' that were recorded and which the director liked.
