It's a common trait for the elderly.

Many elderly, demented women will be quiet and polite when surrounded by family in a hospital, interacting with doctors and nurses and medical aides.
But, within SECONDS of that family leaving to go home (or even just to grab a meal), that same demented elderly woman throws an emotional riot and begins swearing, screaming, howling, scratching, slapping, spitting, and biting anyone who interacts with her. Hell, especially if no one is interacting with her, she'll scream and shout and cry and hurl herself out of bed and do anything she can to attract attention from absolutely anyone... and when you respond, she'll hate you for it.

Elderly people genuinely behave like this.
