The Philosopher remnants still exist, and they are pissed at their stolen money so they're out for revenge and to recover it. A faction then forms the Patriots. And MGSV would have been a WAY more interesting game to bring back Eva for.

Ocelot is just pretending to be people… for reasons. Any vaguely mystical about the previous games… nano machines. In fact, any lingering plot point Kojima doesn’t know how to resolve… nano machines. It fucking sucks.Again, Kojima did the MGS2 shit was 4; he couldn’t just make a proper conclusion. The first trailers had to let us know Snake is old and everything went to shit and he put a gun in his mouth and wants to kill himself. Great way to show your hero in his return as the main character of, what was going to be, the final installment of the series. It sucked then and it sucks now.

Spend the next few games telling Big Boss' rise and then explain the origin of the Patriots. The English MGS official guidebook took some of the canon plot points and then created a fan-fic biography based off of them, and actually wrote a more interesting backstory than what Kojima gave him. I bet you anything they never thought that Big Boss, who was dead, was ever going to ever appear in a sequel, so none of this mattered.
