It feels endlessly replayable, you always unlock more stuff that winds up inside your runs. It plays like a shoot 'em up crossed with a legend of zelda dungeon. Think Starfox in that you advance through levels and have the choice between different branching paths. It can take a 1000 hours to fully complete the game, by which time you will likely have beaten it several hundred times.

it's all about the items. there's a lot of them, too many to keep track of, it always keeps runs fresh, you'll always discover new combinations, it never gets boring
at this point though after like 5 DLCs one could argue there are too many items, and too many of them suck
i wouldn't recommend isaac for a first timer in 2025. play something like spelunky instead

Most roguelites don't have the sheer volume of gameplay mixing that Isaac has with the powerups interacting with each other. There are so many god damn combos for you to find and have to completely break the game, and then there are some that completely ruin you because of some specific interaction. It's been a while since I played though so who knows if those run-ending items have gotten fixed in some way that makes them not as horrible. Fucking onions milk.
