Because the monsters primarily exist in the spirit realm, the impact they have on the physical realm is deliberately ambiguous. In one specific ending, Hinako starts perceiving humans as the spirits and goes on a mass murdering spree. The redness of the corruption IS an expression of Hinako's pill addiction, but it's later stated Hinako herself has an abundance of divine power so the implication seems to be she's subconsciously shaping the otherworld to her will
