pretty sure this is mine, from Crusader Kings 3. There are so many achievements that basically amount to "play [new DLC] for longer than 5 hours" it's easy to get random really rare ones people just don't bother getting because they've put 1000 hours into the game but never touched anything other than Haestein or the Holy Roman Empire
