They could have easily gotten out with far less deaths if Makoto wasn't a passive bitch. There's so many ocasions things only got as bad as they did because Makoto was a retard (Sayaka) or way too passive (Chapter 2 motivations and Sakura's situation). Its not just Makoto, obviously. Makoto wasn't the only one being too passive, but as the Main character, it's the most obvious. Makoto being the way he was was actually a detriment in my opinion. It worked for a very specific scenario, but that scenario was far from the best that could have been achieved.
