It's nothing extremely overt but it's noticeable and sometimes makes things worse. I think recently the patch lowers lockpicking difficulty on some doors because the plus version nerfs blood buff which makes it less useful for lockpicking, but the change wasn't put only into plus. And it wasn't that long ago it was making changes to music which pissed a lot of people off. I'm not quite sure how it changes Disciplines right now but I remember it letting bosses be stunlocked before when that wasn't possible in vanilla.

One thing they really need to patch in is the ability to go visit NPCs and chat after major story beats. We should be able to have a discussion about certain actions that might need to be taken for the city's future. We can't speak to Ryong. We can't speak to Tolly. Adding extra conversations at key story beats would do so much to take it feel more fleshed out. There's a minimum viable product energy to a number of key scenes.
