In a fictional story, a character's appearance is an incredibly powerful tool. Mitsuo looks the way he does so that you're put off by him the second he appears, even before he says a word. I believe the intention was to have you reacting to his behavior the same way the characters were, and his design is a sort of trigger to make you as repulsed as they were.I don't think there's a deeper meaning to it, but if he were handsome it might create a dissonance where your reaction is completely different to the characters.
