DDLC is a pretty bad example of this. All it does is creating files in its own folder (or rather, extracting from the rpa archives) and look up your PC name. You wouldn't even notice if you use shortcuts or the search bar, until Monika tells you

The very first version of Deltarune chapter 1 was technically malware. The uninstall would delete wantonly, so if you installed the game in a very stupid way the uninstall could hurt your computer.

The silly shit they do doesn't even require admin permissions.You should be worried about the ones that ask kernel level access or want to run hidden processes 24/7 with the pretense of anti-cheats/DRM or whatever. As for registry, actually most programs on your PC do that in one way or another, it's not a huge deal.
