The game loves to mix the literal and figurative, so I think the birth theme has more to do with spiritual rebirth. Part of getting to the white river involves fusing with the sky fetus as the death bell tolls, and once she's there ENA is literally hanging out of her own guts. The Bathroom, a place of bodily cleansing, is treated like a holy site and analogous to a church, a place of spiritual cleansing. It seems less like ENA is making a new entity and more that she wants to be freed of whatever sins she's committed and born again as a new person. It also ties in with Theodora's comment about "nobody deserves punishment for being born, except you." Maybe ENA's sins are too grave, maybe her punishment is incomplete, or maybe she somehow cheated the process of seeking absolution.
