Why are horror/grim games so completely cowardly and sanitized now? Even so called mature ones.
They shy away from sex, sexual tension, actual human interactions, rape.
Its not a fetish thing, its about tone.

Horror games devolve into quirky mascots with catchphrases playing peek a boo over and over, ans they can be killers and horrible people/things but there's a Barbie groin in ever single one.

The nature of copyright is to consolidate publishers into a much smaller handful of culpable entities who can easily be pressured by the government, interest groups and either of those two acting through the other or in concert.
A lot of the sexually charged horror of the past has been sandblasted into inoffensiveness or erased over time and publishers will not touch new attempts at it.
Human creativity is based heavily around rapid iteration, so stifling and curating iteration steers that creativity away from specific topics the government and interest groups would rather that you do not discuss.
Quirky horror mascots see a lot of rapid iteration, but they are themselves gated by their indiscriminate audience and rules for advertisers that their chosen marketers (streamers) adhere to.
