Nero being genderbent isn't as interesting as Artoria and her being a saberface is unnecessary, yes, but at least she's still a great character in her own regard in her games. It's not Extra's fault that Grand Order decided to take inspiration from only its weakest aspects.

The backstory for Extella is that the events of Extra involve MC taking Nero, Tamamo being your Week 3 caster opponent, and the enemy master basically throwing Tamamo under the bus so MC took her on as a second servant. The whole "split into mind, body, soul" storyline of Extella is the reason that you have three MC and that Nero, Tamamo, and Altera all have one.Maybe you've mixed things up a bit with the Fate/Extra: Last Encore anime, where there was a female MC of Nero in flashbacks and the current story was following male MC with the same Nero.
