If you play on the as-intended Expert difficulty you can just barely eke out wins dueling against some literal who lvl1 100hp farmer armed with an iron dagger. You need to approach the game like KCD or Gothic. Assume you die to anything stronger than a wolf and spend a bunch of hours on fetch quest, exploration and non-combat skills to get XP on money. It'll be a loooong while before you can take bleak falls barrow by yourself. After playing through ~200hr of a normal-mode playthrough, I tried hard mode Nefaram which is a hellish experience and even worse because you need to pay out your ass for loicense after loicense after loicense. It's playable but it makes it practically mandatory that you know the modpack in and out and how to abuse things in your favor.
