Only the important story missions.
The game doesn't expect you to save every survivor or fight every boss in one playthrough.
There are a few different endings and leveling carries over between playthroughs too.

Fond memories of the original, though I never cared about the missions or rescues to the point where'd I'd trannyrun the game and sweat about it.
