This. Anything can become an insult. "Nice Driving" on a racing game could mean someone did something good but if you only see it get spammed during a crash then suddenly it is the goto insult. Bloons Battle games laughing emote or any emote for that matter basically only exist to piss the other player off and do some mind games with. So can't use pictures either. If you only allow letters people will start spelling out awful things. If you only allow basic shapes people will start learning Morse code just to say "fuck you" if not worst. Hell don't even need to know Morse code as stated with Bloons example, in real life the best way to make a traffic jam so much enjoyable is by honking your horn nonstop. Guarantee to speed up the process without pissing anyone off! Hell in real life you can literally just look at somebody wrong. Same apply in video games, t-bagging exist because we allow it to. It literally just crouching. Hell having a player avatar run around another could piss someone off. If you wish for a platform/game/life with nothing offensive then you would need to start with 0 people and keep it that way.
