The escort mission level in Spartan Total Warrior. I swear that single level took me longer to beat than the entire rest of the game. It's the single reason why I never replayed that game on hard, I don't even want to know what it's like there.
Not really niche, but the Courthouse mission in Wolfenstein 2 was the hardest part of any FPS game I've ever played. It's stuck with me since playing that game.
