If I remember right, you need Bloody mess or evil karma to get that. It's not a choice but your character does it automatically.

It's kind of a "good" ending of sorts and justified. You saved the vault, but you iced the Overseer, and now the vault can decide what to do next. I know there's canon explanations, but I do think it's possible that the Vault could see this as a murder and order you to be arrested and this could cause problems.

The correct course of action would have been: Hey listen need you to come in and say some goodbye's you see you're a hero but what if you stay? The harsh actions of the wasteland has changed you, I can tell. What if those traits follow you into the vault? what then? Can you live with that? I think we both know what needs to be said and done.

In the later I heard the US was essentially boiled down to a hundred million give or take. With those living in the setting essentially being the lucky ones that didn't draw the irradiated straws. Fallout despite Todd and co's retardation still had potential for civilizations growing and thriving outside of the main areas if you count OWB in HoI4 as a starting point. Vast enough where cottage level industries can rise in the wake of the industrial collapse but still enough raw materials to reach pre industrial capacity. See NCR and the various entities within it before they got genie bottle'd unceremoniously.

The more I read on the background of Fallout lore pre-nuke, the more I'm convinced that the nukes were a salvation.
