It's a combination of really good character design, a unique art style due to its color scheme, and a well-executed toxic sibling dynamic that you rarely see in media. There are plenty of incest relationships and all, but they're either just between side characters, or have to censor/copout with "not blood-related", or they're in a hyper-idealized and unrealistic relationship where they don't act like siblings at all. TCOAAL really nails the toxic sibling codependency dynamic in ways that lots of other games don't.Also, another underrated part of its success is that it also panders to women. People constantly focus on Ashley and Renee, but Andrew genuinely appeals to women. Literally look at any video of a female streamer playing TCOAAL and there's a good chance they're drooling over Andrew.
