An internal culture that dismissed any criticism because the games did well and made them feel infallible. And over time that circle jerk caused them to completely shit the bed because they never questioned whether a design decision was retarded or not. Seriously when you listen to some interviews with bethesda devs they come off as so incredibly disconnected and sheltered from criticism, they were told their shit don't stink their whole career.
