I think the Pthumerians were a wildcard that every faction could make use of: we see them as Church Enforcers in Lower and Upper Cathedral Ward; there's Pthumerian Giant in the Nightmare Byrgenwyrth Lecture Hall, and Pthumerian kidnappers take you (and at least one member of the Healing Church) to Yaharghul Gaol.
They probably all came up before the split between Willem and Lawrence.

At first there was Byrgenwerth. They discovered the labyrinth beneath Yharnam and brought back Old Blood to the surface. In their search of the divine, they also raided the Fishing Hamlet to search for clues about Kos. Willem wanted to use eyes/insight to ascend humanity, and warned against using Old Blood.

Laurence, a student of Willem, decided to use the Old Blood recovered for medicinal purposes, and founded the Healing Church. He became its first Vicar, and the first Cleric Beast when the beastly source within the Old Blood transformed him. His mind was eventually drawn into the Nightmare, while his body descended underground into the labyrinth.

Some of the Healing Church Hunters discovered the Great Isz region of the labyrinth, and made contact with Ebrietas there, drawing her to the surface. This faction of the Healing Church became the Choir, the group dedicated to creating their own Great One. They're the source of the alien freaks in the woods, and eventually succeeded with the Celestial Emissary.

Another group of Healing Church scholars took after Willem's teachings more and founded the School of Mensis, seeking to either create their own Great One, or to make contact with another, and made contact with Mergo. Mergo was eager to help and granted the Mensis scholars the insight they requested, but Mergo can't control his own power, and the insight overloaded the Mensis scholars' brains and killed most of them. Micolash, their leader, was drawn into the Nightmare as its host, his own body expired. The School of Mensis and the Choir are arch-rivals, and will actively attack one another.

Iosefka was likely a Mensis scholar who defected. Her blood is found in Mergo's Loft, which is the territory of Mensis. The fake Iosefka is a Choir member, backstabbing Iosefka and turning her into a Celestial Minion.

Yeah to some extent the lecture hall and the nightmare frontier are both part of the Byrgenwerth "zone", it just would have been fun to have a whole lovecraft university campus on the shore of the lake with some fucked up science labs and libraries and whatnot. I tried my best to do a 99 Arc run just to set up the Parasite, but trying to beat Orphan with Executioner's Gloves and Augur of Ebrietas is too much. He resists elemental damage to begin with, and, despite probably being Kin, doesn't take extra damage from lightning, so paper and Tiny Tonitrus are useless.

Fishing Hamlet was before the Church began in earnest, iirc, since it was Gehrman and Maria, the first Hunter and his apprentice, that were sent to investigate Kos.
Byrgenwyrth predates the Church, as the schism between Willem (eyes) and Lawrence (blood) is what lead to the founding of the Healing Church and, subsequently, its order of Hunters.
Mensis were, I think, a splinter of The Choir, the elite of the Healing Church who were doing hardcore delving into the arcane: The Choir had its "Orphanage", Mensis had Yaharghul.
As for Queen Yharnam, it's a little unclear. IIRC, Mergo's umbilical cord can be found in the Lecture Hall in the Nightmare, so we can assume Byrgenwyrth has some contact with her. She likely never dies, if the key item you get for beating her is any indication, so it's hard to say where on the timeline exactly she had her child stolen (she's wearing manacles, and her dress is covered in blood from the waist down: Byrgenwyrth probably ripped Mergo from her womb).
