People who, upon me saying hello and how can I help, start calling me slurs, say that I'm a nazi pedophile, talk about how celebrities are fucking each in the ass at their group home, how they just came out of a meth binge where they walked for so long parts of their feet are showing bone, etc etc. That or families talking about how they have concerns since someone has been screaming outside in the sun to the point they need to go to the hospital for medically significant sunburns, how their kid has been staring at nothing in the kitchen for 3 hours, or how their mom threw a rock at a child because they didn't answer a phone call they never actually made to them.There's something to be said for portraying something in a way that is reserved, possibly even tasteful (completely debatable as to whether video games have achieved this though) so that audiences can engage at least to some degree that resonates with what happens in reality. If you just show it plain to the world as it is, you'll be screamed at for misery porn, exploitation, or being edgy.Again, totally up to you if any of those games depict mental illness in any particularly good/accurate way, but you don't find much success in throwing the full reality at audiences with how dire shit gets.

Also, I'd say a significant majority of people play games to have fun and enjoy what's there. Not to be educated on the depth of mental illness as a whole. There has to be a balance between covering interesting topics in a real way, while still engaging with audiences and not just being called an edgy fuck for putting dark content into your game. I mean, look at games with rape in them. Despite it being a very real issue that can be talked about, games with that topic typically face an uphill battle to not just be written off as exploitative.
