Distract is great in a backstab build.
The problem is it’s a vital component to one and rarely worth taking otherwise. And the odds of getting backstab abilities/passives AND Distract start getting really slim.

I don't think caltrops is even weak, Thief just has so many good skills. Harpooner is one of those odd ones that's weak with Butcher's hook but broken on another class if you have a gun. Until you get to higher difficulties and then it's mediocre again because reflect.

Distract is a good skill at any time.
2 mana to deal +25% damage to an enemy until their next turn is great on its own.
Sure, obviously sometimes you can backstab them without it (and without repositioning in a bad place), but that's less than half the time. And when that happens you can simply not spend the 2 mana.
Better than any collarless spell at least, which is more than you can say about Grill.

In lategame when you're bursting down all enemies round 1 it's pretty good, they don't get the opportunity to move away from the walls or each other, so Distract is just a +25% damage boost against those guys. That's not even getting into backstab synergy. Grill is way worse.

Good point but I'm still not sure if that justifies itself. Most of my thiefs oneshot enemies with or without backstab, and I have been able to just use movement to get behind most bosses.

Going back to my MTG comparison, Distract is for Spikes. It's a solid efficiency improvement when you're playing "optimally", ie. killing everything before they get a turn.

It's not a big deal most of the time, but it drives me crazy when high-speed enemies, especially on the hard path, just get a free 25% damage because my cats apparently back into every single fight.
