Deep Down would have been fucking it. Implement a good lighting system so that lanterns (fueled with oil) have a reason to be used, or you can use spells to light up areas in short flashes, have a variety of environments and monsters and just let me go down. DDDA's BBI is the closest thing. I understand making a game entirely around the concept would be hard to come up with something that would last longer than 10 hours though.
