Pretty fun gameplay, completed it in one day and got the good ending. I'm yet to get the other two and also play the second game. Story was pretty decent, and enjoyable, although sometimes I had problems hearing the dialogues between shooting bad guys. AI is straight dogshit but I let it pass since this came out in '03.

the sequel is the better game imo but it has quite a few unnecessary retcons for whatever dumb reason. it is also full of cheap juimp scares but they never really do any damage to you so whenever that happens just keep going anyway.

I remember my cousin had this game, just the box art (not this one, version with bigger needle eyes guy) was enough to scare me as a kid
Now I just have to replay it

The Suffering is one of my favorite games from the 6th gen. It's an absolute shame that this and The Ties That Bind have never gotten a re-release for any modern platforms.

One of the few memories of the game I have is opening an asylum cell and seeing a squirming body with no legs or arms so I shoot it and get a good karma flash of the guys wife saying "you're a good man"
