There were volcanoes, they were very far behind but it's still stupid. Dark Souls 2 has a lot of nonsensical map layouts, some even more egregious than this but the "windmill in the middle of a plain with a mountain range far behind" -> "hallway to an elevator" -> "top of a volcano" was by far the most noticeable and immersion breaking transition of the lot.

On my first playthrough anor londo is where it feels like I was walking through a half finished level. After that it gets worse with an apex of shit when entering lost izalith and having 10+ dragon butts aggro on me.

Those are the obvious ones. What I dont see people talking about is the "go kill 4 niggas" quest isnt as narratively compelling as ringing the bells to access sens/anor londo. The latter implies a journey and exploration, you are tasked to go do something so that you can travel somewhere else. The former comes across as a very arbitrary checklist. If you think about it with a bit of finaggling, you can cut the second half of the game entirely and just have the lordvessel open the path to the kiln right then and there, and it would still be narratively satisfying.

The whole path from Majula (hell, actually right from the starting point in Things Betwixt) to the Bastille is a total mess, transition-wise.
