Sure, the playtesting doesn't help, but fundamentally this kind of thing is just necessary in the kind of industry where you're working with hundreds of other people from inside and even outside the company that will continuously add to, contradict, change, remove and otherwise overwrite your work.
Signposting needs to be obvious to avoid being completely scrubbed out and it can't depend on any prior trends or design language that may be changed or removed, hence something like yellow paint which is obviously a signal.

I'll be honest with you, I'll take yellow (or whatever other color) paint over having walk around scrutinizing with a magnifying glass for hours stuck on the level if the level designers are so incompetent that they literally can't think of any other ways to avoid this.

And if anyone says it never happened to them in Doom, Heretic, Hexen, etc. then they are lying.

doom 64 i really had some trouble. a lot of switches opened doors that were nowhere near, kinda annoying

Good for you, I think it easily gets annoying because running around aimlessly, even if I stumble upon secrets, is not fun and I'd rather do the fun part that is blowing up monsters.

Happened with all old-school Doom games, maybe not so much in 2 and I also remember getting stuck in 1 more, but definitely did happen there as well. 64 is by far the worst offender.
