I'm guessing it's the past, since you see the ruins of the entrance getting fixed in the lake reflection. Since the land in DkS2 is very much the same as DkS1 (and 3), the outside world aspect of the intro is a bitty iffy. It's not completely isolated, since the Dark Sign and the Hollowing still exist in this outside world (you got the dark sign before stepping into Drangleic)

All the souls games take a lot of inspiration from the mystery religions and mythology. A very well made and interactive alternative to the initiation ritual. The journey of the human soul before or after life. The hero’s journey. The big question is who is this hero of a thousand faces? Who Are You?
