Attack monsters to build up charges on the barrel, and spend charges by shaking the barrel to craft unique alchemy items that have different effects. Most are useless but there's a couple decent ones. Alchemy also has exclusive access to leveling SP mode, which can be shared with other hunters in the same zone. SP2 increases stamina recovery speed, SP3 increases natural health regeneration, and SP4 increases the chance that monsters will flinch on hit.
