Him making KOTOR was essentially him, a tech-savvy dude, growing into a Director role in post Neverwinter Nights BioWare and deciding "All this RP slop kinda sucks" and KOTOR is only an RPG because of the well oiled machine he had to work with. Almost every impulse Casey had was "more AAA standard than the last game we made" and he is still doing that.
