It's implied he's responsible for the Blood War starting in the first place, somehow, although it always seemed to me like a bunch of different factions of equally evil demons that hate everyone else would be destined to fight each other forever anyway.

Whatever the fuck his original incarnation did aeons ago disrupted this balance seemingly permanently and the universe is now destined to slowly suffer entropy until eventually everything is gone. Granted this will take an extremely, absurdly long amount of time, but that still makes him the biggest fuckup in existence.

I always assumed it had something to do with how he became immortal. Probably broad experimentation on souls, to the extent that he was able to give the gods a bird and but avoided the fate of the Wall of the Faceless.
