because it has some of the most overtuned "dynamic difficulty" ever conceived, so that if the game detects you doing well and acquiring a lot of items (ammo or health kits) it will artificially whiff your inputs and force you to take damage you otherwise wouldn't have by increasing your own model's hitbox, while randomly raising enemy HP/resistances, weapon accuracy and collision, and dodge movesets
the EXACT SAME AREA can have wildly different enemy behaviors and efficacy based on your current health and inventory, and how well or badly you performed in your last playthrough - it is intentionally designed to disaffect and trick skilled or knowledgeable players to mimic active AI, but in fact it's just flat stat-boosting and bottlenecks

horror games are almost never hard in the traditional dark souls sense of dying 20 times until you finally get throug. they're usually tuned for death to be a danger but not a trial and error guarantee.
I would say that 4make mostly stayed within that framework until the island, on hardcore I did begin to have to approach those machine gun pig bastards like dying in souls.

hardest thing about this game to me was the shooting range challenges. i used pistol for everything except bosses where i would use the big boy weapons ammo on them to effectively skip them like i was playing an old "survival horror" RE from the 90s where you use pistol on everything but the boss fights.
