A big change, for one.

Overall, there are less phases to everything but dodging and staying alive is more difficult. The stages themselves have less "enemy jumps on screen and dumps firepower everywhere" as you get a bit to react to enemies, but they'll typically have more health so you need to focus fire on them to avoid dealing with their attack pattern. Enemies will also constantly use their attack pattern all the way down the screen, rather than just appear and use it once. It's more about determining which enemies are the worst and taking them out first, as opposed to just remembering where everything shows up and shooting that area of the screen to avoid the enemy from attacking as soon as it shows up.

Bosses have much, much distinct phases in CAVE than in Touhou. The ones you do face tend to be more complex. While Touhou tends to have a bunch of phases with a pattern that has a different gimmick each time, the CAVE bosses tend to have patterns which overlap and the challenge is dealing with each one. While CAVE bosses have phases (sort of), they built up upon one another and so the second pattern is generally a larger or more challenging version of the first, rather than just completely independent ones.

There's also some times when you just need to move out of the way or need to be highly mobile, as opposed to just trying to slowly move around and squeeze thru gaps like it the majority of Touhou patterns.
