One of the first examples was Far Cry 2 where taking out a check point could suddenly have patrol vehicles drive up and start attacking the player, while having an ammo box blow up and set grass on fire and just make the entire situation more chaotic. It's basically just a fancy way of saying, "Not scripted or set encounters."
