I think what people forget is that
1. The MC usually has a change of heart about wanting to return home midway through
2. They're supposed to be forcibly flung back to their old world at the worst fucking possible time
3. The MC has to come to realize his experiences in the new world have irrevocably changed him and made him no longer fit in his old world and he sever old ties like family, old lovers, punch his boss, etc. Before reclaiming his life in the new world.
It's never meant to be the end-point, it's part of the isekai journey to return and either bridge the two worlds or realize you are no longer part of it.

Because it depends on what your life was originally. If you have people that love and care for you then even if you had power and safety would you truly drop them like a bad habit, knowing they will forever think you died, ran away from home, abandoned them, or so on?
If your life in the original world was filled with abuse and all that then yeah, it's a no brainer to stay in the isekai world.
If your life in the original world is just average, you don't really have anyone who cares for you to such a degree they'd be devastated if you disappeared, but you do have people who miss you then it's really up to you at that point.

There's also the issue that in a lot of these isekai plots they're going to a world that's either filled with constant violence from bandits or monsters that you are regularly having to deal with, or you're going to a massively technologically-stunted world that at best lets you control some elements. The worst you have to do right now is work a dead-end job to pay for your living situation, get food from a building that is nearly always stocked, have fresh water on demand, have heating and cooling on demand, actually have something to do to pass the time like play video games, watch TV, or browse the internet without perpetually being chained to a local tavern, and so on. Now you go to a world where every day you have to worry about homesteading just to keep yourself fed, preservation using cellars or figuring out how to apply frost magic to preservation so you don't die for three months of the year, no sanitation whatsoever, having to do your laundry in rivers, having to rely on manually heating water either with a fire or magic just to have warm water, etc.

Unless you are The Chosen One™ who gets the masses to provide you with your needs you are going back several hundred years in your quality of life, and you better be an exceptional ruler or you're gonna have major political issues arise very quickly.

Unless you speedran killing the demon king or whatever it is you need to do, your experiences in the isekai world would've changed you as a person so much that you wouldn't be able to connect with those in your previous life anymore. It's the same reason veterans have so much trouble reintegrating. Also if time passed in your original world then any loved ones or friends you had would've already grieved and moved on. Your posessions would be sold or given away, your job or school would've dropped you, if you had a girlfriend or crush they would've found another, and so on. So not only would you not be able to connect with anyone anymore but you would have nothing to return to anymore. You are effectively starting an entirely new life, except people you once knew can say stuff like "He was never the same after he went missing".

It entirely depends on what is going on in your isekai world. Sure, it's going to change you but it wholly depends to what degree you are doing stuff because killing monsters and killing people is going to have a different mental effect on you, especially if you are The Chosen One™ and have some sort of special abilities that let you disassociate or significantly avoid situations that would cause threats to your life in a serious way. It also depends like you said on if realtime passes or not. If it doesn't then fundamentally you will change personally because to you it will be six months, a year, or even multiple years, but to the people in your original world it might have just been a second for every day that has passed.

Honestly, there's way too many factors about this to get mad about it one way or the other, and not everyone is going to be that guy that gained massive stress lines in WW2 despite being like 20 from being isekai'd, and not everyone is suddenly going to 180 on their personality because they became The Chosen One™. Even in the isekai manga and anime they usually don't change that much as people. At best they become experts in some field more than anything and gain a bunch of prestige.
