The M4 and most related rifles have the bolt lock open when empty. There is a little tab on the magazine follower (the plate that pushes the bullets up) that when it is fully up trips a little catch. There is a button on the side of the rifle that bumps the bolt off that catch so it slides forward again, which you do after you insert a loaded magazine. This turns rechambering the rifle from a medium dexterity slow process (find and pull the charging handle) to a low dexterity quick one (slap a button).

Those mechanisms are a little different. Look at this picture. See that little round boy on a pivot? That is the bolt release. When you shoot the last round in a mag, the bolt automatically locks back, swap the mag, press that button, and it's ready to fire. You see people slapping it when rushing to reload and to make sure you press it hard enough to release the bolt, so a sharp rap with the heel of your hand and you're back to shooting.
In the pic you are replying to, that part is called the forward assist. You don't pull it. If the bolt gets gunked up or if it cycles too slowly it won't be far enough forward to lock the bolt in place and won't fire. If you pull the trigger and it's locked, you hit the forward assist with the heel of your hand and it it supposed to shove the bolt into place. In theory, anyway. In practice it just jams the fuck out of the bolt and makes even more problems with trying to get it to fire. Either way, it's a button you push, not a rod you pull.

It makes me have a lot respect for how DICE handled the guns in BF1. They got the Forgotten Weapons guy in just to help them learn how a lot of those guns they put in the game were meant to reload, so they got them to reload as accurately as they could. The Lebel rifle in particular is a funky gun that reloads like pretty much no modern bolt action rifle proceeding it, but they still went through the effort of getting its reloads right.

They are still there but they have been getting smaller and out of the way or there very well could be some models that don't include them, i don't know every gun ever and there are a lot of people kicking out AR-15 platforms these days. They really weren't necessary in the first place. They gun can run just fine while dirty. Not submerged in mud fine like the AK, but it can still run fine dirty. I don't know the exact story, but I think Stoner didn't include the forward assist on his first designs but the military WHAT IF'd him into putting one on. Or something. Forward assist has kind of a weird story including fringe cases that just never come up so it's ultimately useless. The only time you'll use it to knock the bolt in is when you're playing with it really quietly and don't want to wake mom up with a loud CLACK of the bolt slamming forward. Past that, a jam is probably because of a stove-pipe, a failure to eject the casing properly, and slamming that forward assist with a store pipe will just make the jam worse.

Because of this, the AR-15 is pretty much the fastest-reloading rifle design ever.
As a bonus, with a 20" barrel and a rifle-length gas system, it has basically no recoil or muzzle rise, and it gets even better if you hang a noob tube off the front, albeit heavier. It pretty much just vibrates in place.

There was one thing that always stood apart on the guns from Leviathan in D2 and that was the fact that they had radiolaria in their chamber. Radiolaria is basically a key component of the Vex faction which has mastery over various things like space, time and simulated universes so the guns that utilize any of that substance might as well work on magic. I think the biggest mistake they made was still make the Leviathan raid guns spit out casings, as there are plenty of other guns in the game that actually don't and could technically be considered to run purely on some form of energy propellant. No Feelings was one such example, with the design still incorporating a bullpup energy magazine but ejected no casings upon firing.

This is made even funnier by the fact that bethesda went on to make a double barreled shotgun in starfield that has square chambers, and shoots bullets. You don't "slap" the side of an M4, if you have loaded a new magazine into the lower receiver, you pull back on the charging handle to load the first round into the chamber. If you're loading a new magazine in, you hit a press that sends the bolt forward, which also loads a new round in the chamber.

You ever wondered why shotguns have two cylinders, one on top and one on bottom? It's because the bottom one stores the shells, and the top one actually fires them.
Actually shoving shells into the firing cylinder wouldn't work, but if you perhaps had an open storage cylinder, then you could just put shells in through the front. The problem is that inside a normal storage cylinder, there's a big spring that helps push the shells backwards into the firing cylinder that won't be there if you have an open storage cylinder.

It's elite ball knowledge, to be fair. Off the top of my head, the only game I've seen this portrayed accurately in has been the M16 in BF3 and BF4 (don't know if they kept it in BF2042 or BF6, I didn't play them). Most other AR slaps are still aiming for the button when you analyze the animation, but in 3 and 4 you can see that the hand explicitly DOESN'T hit the bolt catch. DICE cared a LOT about having really good gun animations in that era of BF.

QA doesn't okay anything, QA sends it's findings depending on how big they are either to the devs/animators/whoever is in charge of creating the part of the product they're analyzing or directly to corporate

it's then those people that decide if QA's findings will actually be fixed or if it's too expensive to fix and their findings will end up buried in some map nobody will open ever again

And that's only for big things like the bullet casing thing, for smaller things the average QA team has on average the budget to do about 5% of the work they should be doing so things slip through the cracks

IIRC what's happening is the soul can't move on to the afterlife because it can't break the connection to the body it was in; I can't remember the exact details of necrosis but basically it's caused by the soul trying to get back in, which it can't really do because the body's not alive. The left handed bolts wouldn't bother me if they used their gunsmithing system to allow the player to flip it, but they didn't. The ISA rifle in Killzone is some weird bullpuperized AR-15 that doesn't really go together. AR pattern rifles have a spring that goes all the way back into the stock so you can't really just bullpup them, and then the controls are otherwise not changed which means the forward assist (the huge button on the right hand side) is kind of just floating there. Normally the FA interfaces directly with the bolt to push it forward and form a seal under adverse conditions but it can't do that here when the bolt would be waaaay further back.
