I imagine games like the next GTA are more of the former, where developers have such ridiculous ambitions for the game that just make development monstrous. According to people at Square Enix, games have become basically as complicated or more so than movies, requiring in addition to the actual dev time, a lengthy pre production phase to plan and map things out as best as they can, since changing things drastically partway through will be a massive pain in the ass in modern games. They also essentially spell out that the complexity is so extreme that it can't be reduced even by just throwing more and more people at a project.

In the future, a new profession will be the AI CEO. As a neo-CO, your job is to manage the 20 AIs. I mean 18 AI and 2 people punching the AIs when they do something wrong. These AIs will be coordinating together to make up for the peoples.
