Generally medieval cities were relatively small, tight and insular cultures and communities, and a sort of lower elite (compared to aristocracy) with their own rights and privileges and strong civic identities, and if you are making their ward/quarter/street dirty people will know or soon work out it was you, who you are, your family, your profession, your local officials, and they would make it a problem that you degraded the neighbourhood, as you note in some cities you might have given them legal trouble or financial penalties

For me, it's just the fact that modern fantasy never does anything outside of medieval Central/Northern Europe. There's very few games based on ancient Greece,Rome, Spain or the Balkans and their myths. Even less on the Eastern Roman/ early to mid medieval counterparts on those regions and next to none on based on the Asian subcontinent outside of Japan and China (and even then they're few and far between).

Frankly, modern fantasy is the most uncreative genre and the least interesting setting for a game second to the modern world (barring games that mix the modern world with mythological/sci-fi shit). It hasn't changed since Tolkien died and it's going to remain that way forever.

Don't forget people back then had half the work time they have today, likely less. There were almost weekly celebrations and living together was actually a thing. People cared about each other. When statistics talk about low rates of literacy in those times, it talks specifically about literacy in Latin, which was indeed low, as most people had no need to learn it.

For peasants, keeping their teeth clean was very important, as teeth could be sold to the nobility if times were tough. Nobles ate sweet foods which eroded their teeth. For some good money, peasants would sell their good teeth which would then be implanted in the mouths of nobles. Peasants would wash their teeth with salt, or if it wasn't available, then just rinse with water after meals.
