Despite everyone fighting at all times there is no "war" in this universe(yet), so an aerial assault is unlikely. Chances are the fortress was built back in an era where aerial attacks weren't really a thing either. There are multiple fortress walls so an artillery doesn't seem like a huge risk, they aren't going to hit the inside.Also its based on an actual fortress I'm pretty sureActually when you really start looking there are dozens of cities with a similar design to this I think.
