In some cases, realism makes for a more interesting story cause there are more tangible consequences that lead to more compelling stakes and reduce the suspension of disbelief. In the case of romance, the right amount of realism thrown in makes or breaks the difference between trashy wish fulfillment slop or a good romance story. I feel this applies more so for sex scenes as some are just so over the top it ceases to be hot and makes you roll your eyes. It's all about finding the right balance.
