The "ditto face" was an anime invention for the storyline of that particular ditto that had trouble imitating faces. Now it's all over the place, even in the official fucking games applied to things you wanna visually imply is a transformed Ditto. Another example of the cancerous tv show encrusting itself to the rest of the franchise and shaping it's future.

in the first two gens a transformed pokemon would retain its original palette. you could take this to mean a ditto could perfectly replicate the shape of anything but would still be purple goop.
Every single Ditto is supposed to have their own imperfections, even if Game Freak was a large AAA company they wouldn't bother accurately representing that.
