Any suggestions on what to name the streams? I'll try my best to be around for all matches so we have them recorded, but figured it might be worth it to give them proper names or even make each match its own stream by quickly turning the stream on and off.

Its the same, just that instead of OBS, PRISM live handles the game stream. You can duplicate the phone screen to the pc and still have the same experience.
If we are hesitant on discord, something like jitsi also works.
