The only way to continue in Twisted Metal 2 is to enter the level password, which is shown during the loading screen. There is also a password for starting at Dark Tooth boss fight, but is that password actually shown anywhere ingame? Was it just in the official guide? Is it cheating to skip the Hong Kong level and go directly to Dark Tooth if you lose against it?

I can't beat Antarctica or Holland without taking at least one death. There's just so much open ground for Spectre to hit you with his bullshit missile and Sweet Tooth to constantly spam his special.
