Critical Point, a late 90's Visual Novel about sex rabies in space. It's actually pretty good and still available digitally to this day.
I couldn't find a box shot for it with the AO rating visible, but ESRB website does list it along with some other Peach Princess titles like the infamous Water Closet: The Forbidden Chamber which is likely the worst game to have ever received an official AO rating.

Kichikuou Rance. Once you learn how the game works it holds up really, really well. This was going to be Alicesoft's swan song and they went all-out on it, but it was a massive success and saved the company, and it directly inspired the rest of the games in the series up until its conclusion.

It was dry humping at worst and every single aspect of the minigame was in the game, all the "mod" did was enable it.

Amusingly, the news had no problems showing the minigame's content, yet the news is not rated X.

I had a lot of fun playing Steam Hearts back in the day, though I had the Saturn version which was censored since it was released after SEGA started banning AO games on the console.

What the fuck? This game of all things was re-released on Steam? And it's not only based on the censored Saturn version, but they censored it even further while just emulating the Saturn version in a shitty emulator?

Similarly to the low number of AO-rated games, a tiny number of films have ever been rated NC-17 by the MPAA.
Most port doesn't get submitted for rating because it's a foregone conclussion.
