Aside from the MMOs that failed almost immediately, the only classic MMO I can think of that's dead dead is Asheron's Call. That said, most aren't in a state that you'd actually want to play...

There are opensource private AC servers that run surprisingly well. Sadly it is a reconstructed experience, but they've done a shockingly good job on it just based on old datafiles and logs. It's quite playable, even dungeons and most quests and such.

To follow up, a great thing is that because Turbine was one of the few companies to openly allow pretty much all forms of modding, including autopathing and item scanning, huge amounts of data on all kinds of game systems were able to be recorded before the official game shut down. While the actual server netcode is reconstructed, it uses the original client and most stuff "just works".
I think the one standout thing that was lost is a lot of NPCs seem to have reset to default faces. But otherwise monsters, loot tables, shop inventories, quest dialogue and triggers etc, spells and special effects, most things seem functional. Even the player housing works, and they took out monthly rent payments so you can keep your house forever.
