Achi gave Brad her blood. He kills himself to prevent his transformation.

Brad gave Kachua his blood. The blood's latent abilities awakens when someone is close to death and transforms them into a biological weapon. Kachua transformed to the massive rampaging monster you fought in the second stage. Since Saki has also been given Achi's blood, he also transformed to fight her.

This is a game I kept reading about since the early 00s and then when I finally got to play it it met and surpassed expectation. Just splendid.

same for me. I played the sequel first despite being a Treasure fan since Mischief Makers. for some reason this game was really hard to find when it released on the N64, or maybe I just had the shittiest luck. I even got an NTSC-J converter cart just for this game that went unused for years lol.

Pretty much all Treasure games are spiritual sequels to each other, some more so than others.
Sure, the studio swore off making direct sequels for the first 10 years of their existence, but boy did they love iterating and remixing things.
